氨酚曲马多和氨酚双氢可待因治疗慢性疼痛的疗效和副作用①

Efficacy and Side Effect of Paracetamol and Tramadol Hydrochloride Tablet and Paracetamol and Dihydrocodeine Tartrate Tablet on Chronic Pain

摘要: 目的对比氨酚曲马多和氨酚双氢可待因治疗常见慢性疼痛的疗效和副反应。方法2013 年10 月~2015 年10 月,慢性疼痛患者84 例分为两组。A组(n=42)口服氨酚曲马多,B组(n=42)口服氨酚双氢可待因,连续服用5 d。比较疼痛数字分级评分(NRS)、满意率和不良反应发生情况。结果治疗后,两组NRS评分显著下降(t>9.402, P<0.001),两组间无显著性差异(t=0.461, P=0.941)。两组患者疼痛缓解程度和总有效率组间无显著性差异(χ2=0.979, P=0.323)。两组间头晕、呕吐及疲乏发生率无显著性差异(χ2<1.451, P>0.061);A 组便秘(χ2=5.864, P=0.006)、腹胀(χ2=9.946, P=0.002)明显低于B 组,恶心发生率高于B 组(χ2=4.934, P=0.026)。结论氨酚曲马多与氨酚双氢可待因镇痛效果相当,但所诱发的消化道反应不同。

Abstract: Objective To compare the efficacy and side effect between Paracetamol and Tramadol Hydrochloride Tablet and Paracetamol and Dihydrocodeine Tartrate Tablet on chronic pain. Methods From October, 2013 to October, 2015, 84 patients with chronic pain were divided into group A (n=42) and group B (n=42), who took two Tramadol tablets or Dihydrocodeine Tartrate tablets, three times a day orally for five days. They were assessed with Numerical Rating Scale (NRS), and the incidence of satisfaction and side effect were investigated. Results The score of NRS decreased in both groups after treatment (t>9.402, P<0.001), and there was no significantly difference between them (t=0.461, P=0.941). No significant difference was observed in incidence of pain relief and satisfaction between two groups (χ2=0.979, P=0.323). The incidence of constipation (χ2=5.864, P=0.006) and abdominal distention (χ2=9.946, P=0.002) were less in group A than in group B, but the incidence of nausea was more (χ2=4.934, P=0.026), while the incidence of dizziness, vomiting and tiredness was no significantly different between two groups (χ2<1.451, P>0.061). Conclusion The analgesic efficacy is similar of both drugs, but different in side effects of gastrointestine.
