重复经颅磁刺激治疗脑卒中后执行功能障碍的研究进展①

摘要: 执行功能障碍是脑卒中常见后遗症之一,严重影响患者生活质量。重复经颅磁刺激作为一种新型电生理技术,逐渐被用于探索脑卒中后执行功能障碍的临床治疗。本文就脑卒中后执行功能障碍的概况、重复经颅磁刺激的基本原理和作用机制、其改善脑卒中后执行功能障碍的临床应用和安全性等做一综述。临床研究表明高频刺激、低频刺激、联合其他治疗方法对执行障碍均有疗效。然而,重复经颅磁刺激治疗脑卒中后执行功能障碍的作用机制和最佳治疗方案尚无统一定论。

Abstract: Executive impairment is one of the common sequelae of stroke, which seriously affects the quality of life of patients. Repetitive transcranial magnetic stimulation (rTMS), as a new type of electrophysiological technique, has been used in the clinical treatment of Post-Stroke Executive Impairment (PSEI). This paper summarized the survey of PSEI, the basic principle and mechanism of rTMS, clinical application of rTMS for PSEI and its safety. Clinical studies showed that high frequency stimulation, low frequency stimulation, and combination with other therapeutic methods were effective in PSEI. However, there was no unified theory about the mechanism and the best treatment plan of rTMS for PSEI.
