Effect of physical activity on anxiety for children and adolescents: a systematic review of systematic reviews

摘要:

目的 评价身体活动干预对儿童与青少年(4~19岁)焦虑的效果,以及不同身体活动模式、剂量和人群因素对干预效果的潜在作用。

方法 遵循PRISMA和PRIOR指南,检索2019年至2025年间发表的儿童青少年身体活动与焦虑关系的系统综述和Meta分析,采用AMSTAR 2评估纳入文献的方法学质量,采用定性与定量相结合的方式对证据进行整合。

结果 共纳入7篇文献,其中高质量4篇,中等质量2篇,低质量1篇。规律身体活动能降低儿童青少年焦虑水平,效应量为中等程度(SMD = -0.39~-0.54),抗阻训练和低强度运动可能更有效。最优身体活动剂量的证据尚不充分且存在矛盾。

结论 身体活动是一种极具潜力、安全、可及的非药物策略,可用于儿童青少年焦虑的预防和辅助治疗。

Abstract:

Objective To evaluate the effect of physical activity interventions on anxiety in children and adolescents (aged four to 19 years), as well as the potential impact of modalities, dosages and population factors on outcomes.

Methods Following PRISMA and PRIOR guidelines, systematic reviews and meta-analyses published between 2019 and 2025 that examined the relationship between physical activity and anxiety in children and adolescents were retrieved. The methodological quality of the included literature was assessed using AMSTAR 2, and the evidence was synthesized through a combination of qualitative and quantitative approaches.

Results A total of seven articles were included, among which four were of high quality, two were of moderate quality, and one was of low quality. Regular physical activity was found to reduce anxiety levels in children and adolescents, with a moderate effect size (SMD = -0.39 to -0.54). Resistance training and low-intensity exercise may be more effective. Evidence was insufficient and contradictory for optimal physical activity dosage.

Conclusion Physical activity is a promising, safe and accessible non-pharmacological strategy that can be used for the prevention and auxiliary treatment of anxiety in children and adolescents.
