强化髋周肌群力量训练对功能性踝关节不稳的效果

Effect of Strengthening Training of Hip Muscles on Functional Ankle Instability

摘要: 目的 观察强化髋周肌群力量训练对功能性踝关节不稳(FAI)的影响。方法 2015年5月至2016年6月,选取本院就诊的FAI患者40例,按随机数字表法分为对照组(n=20)和观察组(n=20)。两组均进行常规运动训练,即踝关节周围肌群力量训练和本体感觉训练,观察组在此基础上辅以髋周肌群力量训练。共6周。治疗前后分别对两组患者进行髋周肌群力量评估、姿势稳定性评估、星偏移平衡测试(SEBT)评估及坎伯兰踝关节不稳问卷(CAIT)评分。结果 治疗前,两组各项评价指标均无显著性差异(P>0.05)。治疗后,观察组髋外展及后伸肌群峰力矩明显提高(t>3.528, P<0.01);两组整体稳定指数、前后稳定指数、左右稳定指数均下降(t>2.360, P<0.05);SEBT 8个方向的平均距离均增大(t>2.254, P<0.05);CAIT得分均明显增高(t>3.268, P<0.01);且观察组上述指标均优于对照组(t>2.161, P<0.05)。结论 在常规踝关节肌群力量及本体感觉训练基础上辅以强化髋周肌群力量训练,有助于改善患者的FAI。

Abstract: Objective To observe the effect of strengthening training of hip muscles on functional ankle instability (FAI).Methods From May, 2015 to June, 2016, 40 patients with FAI were randomly divided into control group (n=20) and observation group (n=20). Both groups were treated with routine exercise training including muscle strength and proprioceptive training of the ankle joint, while the treatment group received hip muscle strength training in addition, for six weeks. They were evaluated with hip force assessment, postural stability assessment, Star Excursion Balance Test (SEBT) and Cumberland Ankle Instability Tool (CAIT).Results There was no significant difference in all the indexes between two groups before treatment (P>0.05). After treatment, the peak torques of hip muscles increased in the observation group (t>3.528, P<0.01); the overall stability index, anteroposterior stability index, middle-lateral stability index decreased in both groups (t>2.360, P<0.05); the distance of eight direction in SEBT increased in both groups (t>2.254, P<0.05); the score of CAIT increased in both groups (t>3.268, P<0.01). All the above indexes were better in the observation group than in the control group (t>2.161, P<0.05).Conclusion Strengthening training of hip muscle, based on conventional ankle muscle strength and proprioceptive training, could facilitate to improve FAI.
