电流感觉阈值测定在腰椎神经根病中的运用

Application of Current Perception Threshold in Lumbar Radiculopathy

摘要: 目的 观察电流感觉阈值(CPT)测试对腰椎神经根病的临床意义。方法 2014年9月至2015年10月,腰椎间盘突出症伴神经根病患者86例,分别对双下肢相应皮区近端和远端点采用不同频率进行CPT测试。结果 患侧近端和远端CPT评分均显著高于健侧对应皮区(t=1.659, P<0.001);近端和远端CPT评分均异常的占46.36%,仅远端异常的占35.45%,仅近端异常的占18.18%;所有患者均存在CPT评分异常,其中93.02%在250 Hz下测试异常,56.98%在2000 Hz下测试异常,29.07%在5 Hz下测试异常。结论 CPT对腰椎神经根病敏感,不同频率、多点测试能更全面反映神经根的损伤情况。

Abstract: Objective To investigate the change of current perception threshold (CPT) in patients with lumbar radiculopathy. Methods From September, 2014 to October, 2015, 86 patients with lumbar radiculopathy from lumbar disc herniation were measured with CPT in different frequencies (5, 250 and 2000 Hz) on proximal and distal points of affected dermatome of both legs. Results The CPT grades of both proximal and distal points were higher in the affected legs than in the contralateral legs (t=1.659, P<0.001). The abnormal CPT grades counted for 46.36% for both points, 35.45% for only distal points and 35.45% for only proximal points. CPT was abnormal in all the patients, and 93.6% of them appeared at 250 Hz, 57.4% at 2000 Hz, and 29.8% at 5 Hz.Conclusion CPT is sensitive in lumbar radiculopathy, especially in multiple frequencies and test points.
