内热针治疗慢性非特异性颈痛的超声弹性成像研究

Effects of Inner Heating Dry Needle Therapy on Nonspecific Chronic Neck Pain: A Ultrasound Elastography Study

摘要: 目的 探讨内热针治疗慢性非特异性颈痛的临床疗效。方法 2017年10月至2018年3月,60例慢性非特异性颈痛(NCNP)患者随机分为内热针组(n=30)和热磁组(n=30)。内热针组予内热针治疗,热磁组予热磁治疗。治疗前、治疗后1周、治疗后1个月,超声弹性成像测定两组双侧斜方肌、头夹肌、头半棘肌、颈半棘肌和多裂肌应变率比值(SR),记录疼痛视觉模拟评分(VAS)和颈椎功能障碍指数(NDI)。结果 治疗后1周和1个月,两组VAS和NDI均下降(t>2.693, P<0.05);内热针组VAS和NDI显著低于热磁组(F>8.048, P<0.001);治疗后1周和1个月,内热针组各肌肉SR明显下降(F>6.22, P<0.01),热磁组仅右侧头半棘肌SR下降(F=4.35, P<0.05)。结论 内热针治疗可提高NCNP患者颈部肌肉弹性,缓解疼痛,改善颈椎功能。

Abstract: Objective To explore the clinical effect of inner heating dry needle therapy on nonspecific chronic neck pain (NCNP). Methods From October, 2017 to March, 2018, 60 patients with NCNP were randomly divided into needle group (n=30) and magner group (n=30). The needle group received inner heating dry needle therapy, and the magner group received hot magner therapy. They were measured the strain ratio (SR) of bilateral trapezius muscles, scalp clamp muscles, cephalospinal muscles, semispinalis muscles and multifidus muscles with ultrasound elastography before, and one week and one month after treatment, respectively, while they were assessed with Visual Analogue Scale (VAS) of pain and Neck Disability Index (NDI). Results The scores of VAS and NDI decreased in both groups one week and one month after treatment (t>2.693, P<0.05), and decreased more in the needle group than in the magner group (F>8.048, P<0.001). SR of all the muscles decreased in the needle group one week and one month after treatment (F>6.22, P<0.01), and only decreased in the right cephalospinal muscles in the magner group (F=4.35, P<0.05).Conclusion Inner heating dry needle therapy could recover the neck muscle elasticity to relieve pain and improve ceivical function in patients with NCNP.
