居家运动训练对老年肥胖型肌少症的效果

Effects of Home Exercise on Sarcopenia Obesity for Aging People

摘要: 目的 探讨居家运动训练方式对老年肥胖型肌少症的影响及其机制。
方法 2018年1月至4月,80例老年肥胖型肌少症患者随机分为对照组、渐进式阻力训练(PRT)组、有氧训练(AT)组、肌力和有氧律动训练(CT)组,每组20例。各训练组行预定运动训练,共8周。训练前、训练后8周和训练后12周,检测身体组成、肌力和血液胰岛素样生长因子1 (IGF-1)、白细胞介素6 (IL-6)的变化。
结果 训练后,各训练组骨骼肌质量较对照组增加(P < 0.05),脂肪质量、体质量指数和内脏脂肪面积减少(P < 0.05);各训练组背部肌力升高(P < 0.05),PRT组和CT组握力、膝伸肌力升高(P < 0.05);PRT组和CT组IGF-1升高(P < 0.05),PRT组IL-6下降(P < 0.05)。
结论 居家运动训练能改善老年肥胖型肌少症患者的身体组成,提高肌力,可能与增加IGF-1分泌、降低IL-6浓度有关。

Abstract: Objective To explore the effects of home exercise on sarcopenia obesity elderly.
Methods From January to April, 2018, 80 patients with sarcopenia obesity were randomly divided into control group (CONG, n = 20), progressive resistance training group (PRTG, n = 20), aerobic exercise training group (ATG, n = 20) and muscle strength combined with aerobic exercise training group (CTG, n = 20). The training groups accepted their exercise in home for eight weeks. Their body composition, muscle strength, and insulin-like growth factor 1 (IGF-1) and interleukin 6 (IL-6) in blood were measured before training, eight weeks and twelve weeks after training.
Results Skeletal muscle mass increased in all the training groups compared with that of CONG after training, while fat mass, body mass index and visceral fat area reduced. Muscle strength increased in all the training groups compared with that of CONG after training, while the grip strength, knee extensor strength increased in PRTG and ATG. IGF-1 increased in PRTG and ATG, while IL-6 decreased in PRTG.
Conclusion Home exercise can improve the body composition of patients with sarcopenia obesity elderly, as well as muscle strength, which may be related to increase of IGF-1 and decrease of IL-6.
