后颅凹开颅术后重度头痛的病因学分析

Etiology of Severe Headache after Posterior Fossa Craniotomy

摘要: 目的 探讨后颅凹开颅患者术后与神经外科情况无关的重度头痛的发生率和临床特点,并进行鉴别诊断。方法 对119例行择期手术的小脑幕下肿瘤患者进行回顾性分析。结果 术后重度头痛的发生率为37.8%(45/119):神经痛16.0%(19/119),颈源性头痛7.6%(9/119),单纯切口疼痛14.3%(17/119)。结论 后颅凹开颅术后除神经痛及切口疼痛外,颈源性头痛亦是引起疼痛的原因之一。

Abstract: Objective To investigate the incidence, possible mechanisms, clinical features of severe headache after posterior fossa craniotomy. Methods 119 neurosurgical patients undergoing selective posterior fossa craniotomy were analyzed retrospectively. Results 45 (37.8%) patients experienced severe postoperative pain, which could be identified as neuralgia (19 cases), cervicogenic headache (9 cases) and incision of scalp pain (17 cases). Conclusion In addition to neuralgia and incision of scalp pain, cervicogenic factor plays an important role in severe headache after posterior fossa craniotomy.
