腹部加压训练对早期颈髓损伤患者肺功能康复的影响

Abdominal Pressure for Pulmonary Function Rehabilitation of Early Cervical Spinal Cord Injury Inpatients

摘要: 目的 探讨完全性颈髓损伤患者肺功能的特点以及呼吸功能训练对肺功能的影响。方法 外伤所致的完全性颈3~7颈髓损伤急性期患者32例,入院行肺功能测定; 对10例患者进行为期8周的腹部加压呼吸训练,8周后复测腹部加压情况下的肺功能。结果 所有患者的第1秒用力呼气量(FEV1)、肺活量(VC)、用力肺活量(FVC)、呼气峰值流量(PEF)平均小于50%预计值,FEV1/FVC>80%; 接受呼吸康复训练的患者在腹部加压3 kg、4 kg、5 kg时的肺功能指标均较训练前有明显提高(P<0.01)。结论 颈髓损伤急性期患者存在呼吸功能障碍,腹部加压的呼吸康复训练能够改善其肺功能。

Abstract: Objective To investigate the characteristics of pulmonary function after completed cervical spinal cord injury (CSCI) and the effect of pulmonary rehabilitation with abdominal pressure. Methods 32 male patients with CSCI were measured with pulmonary function test (PFT), and 10 of them accepted pulmonary rehabilitation with 3~5 kg pressure on their abdomen 30 min per day for 8 weeks. Results Their forced expiratory volume in one second (FEV1), vital capacity (VC), forced vital capacity (FVC), peak expiratory flow (PEF) were all less than 50% prediction. After 8 weeks, the pulmonary function of 10 patients who received exercise significantly improved (P<0.01). Conclusion The pulmonary function impaired in patients with CSCI. The pulmonary rehabilitation with pressure on abdomen can improve the pulmonary function.
