脑卒中偏瘫患者早期介入减重训练对提高下肢步行能力的影响

Partial Body Weight Support on Walking Ability for Stroke Following Hemiplegia at Early Stage

摘要: 目的观察脑卒中偏瘫患者早期减重训练(PBWS)对提高下肢步行能力的影响。方法60例脑卒中患者分为3组,早期均运用常规康复治疗方法。其中A组20例,整个康复过程均用常规康复治疗;B组20例,在常规康复治疗2周后加PBWS;C组20例,在常规康复治疗5周后加PBWS。3组治疗时间均为8周,在治疗前后分别用Fugl-Meyer、Barthel指数和Berg平衡量表进行评定分析。结果治疗后3组患者下肢功能均有不同程度改善(P<0.05),但B组与A组、C组相比大部分评测指标有显著性差异(P<0.05),C组与A组比较也有显著性差异(P<0.05)。结论早期介入PBWS可进一步促进偏瘫患者下肢行走功能的恢复。

Abstract: ObjectiveTo observe the effect of early partial body weight support on walking ability for stroke patients with hemiplegia.Methods60 stroke patients were divided into 3 groups, receiving conventional early rehabilitation: group A (20 cases) received conventional rehabilitation for 8 weeks; group B (20 cases) received 2 weeks conventional rehabilitaion and 6 weeks partial body weight support; group C (20 cases) received 5 weeks conventional rehabilitaion and 3 weeks partial body weight support. Fugl-Meyer Assessment(FMA), Barther Index(BI) and Berg Balance Scale(BBS) were used to assess the effect before and after treatment.ResultsThe socres of FMA, BI and BBS improved in walking function after the treatment(P<0.05). Comparing with groups A and C, group B improved more significantly after treatment (P<0.05), and group C was better than group A (P<0.05). ConclusionPartial body weight support can improve walking function of hemiplegic patients at early stage.
