电针对类风湿性关节炎的疗效及对血清炎性因子的影响

Effect of Electroacupuncture on Rheumatoid Arthritis: Clinical Outcome and Serum Inflammatory Factor

摘要: 目的探讨电针治疗类风湿性关节炎(RA)的效果及对患者血清核因子-κB (NF-κB)、肿瘤坏死因子α (TNF-α)和白细胞介素1β (IL-1β)的影响。方法140 例符合入选标准的RA患者按治疗方案分为对照组(n=70)和观察组(n=70)。所有患者均给予常规的药物治疗和康复训练,观察组在对照组的基础上加用电针治疗。治疗2 个月后,观察两组患者疗效、关节症状改善情况,检测血清类风湿因子(RF)、抗链球菌溶血素O抗体(ASO)、C反应蛋白(CRP)、NF-κB、TNF-α、IL-1β水平。结果观察组总有效率为92.8%,高于对照组80.0% (P<0.05)。与对照组相比,治疗后,观察组关节疼痛指数、关节压痛指数和关节肿胀指数降低(P<0.05),RF、ASO和CRP 下降(P<0.05),血清NF-κB、TNF-α和IL-1β水平降低(P<0.05)。结论电针可提高RA患者的康复疗效,抑制炎症反应,可能与对NF-κB的抑制有关。

Abstract: Objective To investigate the effect of electroacupuncture on the rehabilitation of rheumatoid arthritis (RA) and level of nuclear factor-κB (NF-κB), tumor necrosis factor-α (TNF-α) and interleukin 1β (IL-1β). Methods 140 cases of RA meeting the inclusion criteria were divided into control group and observation group equally according to therapeutic schemes. All the patients accepted medicine and rehabilitation training, while the observation group accepted electroacupuncture in addition. The effective rate and the symptoms were observed, the serum level of rheumatoid factor (RF), antistreptolysin O antibody (ASO), C-reactive protein (CRP), NF-κB, TNF-α and IL-1β were tested before and after treatment. Results The effective rate was 92.8% in the observation group, which was significantly more than that of 80.0% in the control group (P<0.05). The joint pain index, joint tenderness index and joint swollenness index reduced in the observation group compared with those in the control group (P<0.05), as well as the serum level of RF, ASO, CRP, NF-κB, TNF-α and IL-1β (P< 0.05). Conclusion Electroacupuncture may improve the effects of rehabilitation training for RA and inhibit the inflammation, which may associate with the inhibition of activation of NF-κB.
