全身振动训练对老年人下肢肌肉力量及功能干预效果的Meta分析

Effects of Whole-Body Vibration Training on Low Extremity Muscle Strength and Mobility in the Elderly: A Meta-Analysis

摘要: 目的 评价全身振动(WBV)训练对老年人下肢肌肉力量及运动功能的干预效果。方法 通过搜索 Cochrane图书馆临床对照试验资料库、PubMed、EMbase、Elsevier、Web of Science、中国生物医学文献数据库(CBMdisc)、万方数据库、CNKI等数据库及追溯参考文献,收集研究 WBV训练对中老年人下肢肌肉力量及功能进行干预的临床随机对照试验,按入选标准筛选文献后,提取其中的数据进行 Meta分析。结果 共纳入 15篇文献,其中 3篇文献得分为 5分,5篇文献得分为 4分,7篇文献得分为 3分。Meta分析结果显示,WBV训练组的下肢肌肉力量增加程度高于未进行任何运动训练的对照组,其中肌肉收缩力量测试结果显示 MD=10.11,95%CI 0.46~19.76,力矩测试结果显示 MD=7.98,95%CI 2.60~13.36,差异具有统计学意义(P<0.05);WBV训练组老年人起立-行走测试(MD=1.39, 95%CI 0.73~2.04)和 5 次起坐测试(MD=1.62, 95%CI 0.25~2.98)的完成时间低于对照组(P<0.05);WBV 训练组老年人步行速度(MD=0.02, 95%CI -0.06~0.10)和平衡与步态功能(MD=1.19, 95%CI -0.62~3.00)改善程度略优于对照组,但其差异未见统计学意义(P>0.05)。结论 WBV训练能够有效改善老年人的下肢肌肉力量,增强其下肢运动功能。

Abstract: Objective To evaluate the effects of whole-body vibration (WBV) training on low extremity muscle strength and mobility in older adults by a meta-analysis. Methods Articles were searched from Cochrane Central Register of Controlled Trials, PubMed, EMbase, Elsevier, CBMdisc, Wanfang database and CNKI database, and the reference list of each eligible article were also included. Randomized controlled trials which investigated the effect of WBV on mobility and low extremity muscle strength were included in this article. The methodological quality of each selected article was rated by the Jadad Scale. The data on patients' characteristics, type of WBV intervention and outcomes were extracted. A meta-analysis was then performed. Results 15 randomized controlled trials involving 803 subjects were included finally. 8 were considered to have good or excellent methodological quality and the rest was rated as fair. Meta-analyses revealed that WBV had a significant treatment effect on leg extension isometric strength (MD=10.11, 95% CI 0.46- 19.76), knee extension dynamic strength (MD=7.98, 95%CI 2.60-13.36), and functional measures of mobility such as performance in Timed-Up-and-Go test (MD=1.39, 95% CI 0.73-2.04), and performance in five times sit to stand test (MD=1.62, 95%CI 0.25-2.98) (P<0.05), whereas no significant treatment effect on walk speed (MD=0.02, 95%CI -0.06-0.10) and Tinetti Performance Oriented Mobility Assessment (MD=1.19, 95%CI -0.62-3.00) (P> 0.05), among older adults compared with the control group. Conclusion WBV may be effective on improving the low extremity muscle strength and mobility among older adults.
