运用世界卫生组织残疾评定量表2.0评估老年脑卒中患者整体功能状态

摘要： 目的 运用世界卫生组织残疾评定量表2.0 (WHODAS 2.0)评估与分析老年脑卒中患者的整体功能状态。 方法 2018年8月至2019年2月，老年脑卒中住院患者107例根据病程分为≤ 6个月、7~12个月、13~18个月和19~24个月4个组。入院和出院时，采用WHODAS 2.0和改良Barthel指数(MBI)进行评定。 结果 出院时，患者WHODAS 2.0总分和MBI评分均显著改善(t > 2.481, P < 0.001)。患者WHODAS 2.0总分随病程呈下降趋势(F = 3.444, P < 0.05)，但与他人相处、生活活动和社会参与领域下降不明显(F < 2.410, P > 0.05)；WHODAS 2.0总分与MBI评分呈显著负相关(r = -0.540, P < 0.001)，但生活活动和社会参与两个领域的总分与MBI评分无显著性相关(r = 0.184, P > 0.05)。 结论 WHODAS 2.0可用于评估和分析老年脑卒中患者的整体功能状态。

Abstract: Objective To apply World Health Organization Disability Assessment Schedule (WHODAS 2.0) as a tool to assess the functioning of the old patients after stroke. Methods From August, 2018 to February, 2019, 107 old inpatients with stroke were divided into four groups according to the course of disease: ≤ 6 months, 7-12 months, 13-18 months and 19-24 months. They were assessed with WHODAS 2.0 and modified Barthel Index (MBI) at admission and discharge. Results The scores of both MBI and total WHODAS 2.0 improved at discharge (t > 2.481, P < 0.001). WHODAS 2.0 total scores decreased with the course of disease (F = 3.444, P < 0.05), but no significant decrease was found in the domains of Getting Along, Life Activities and Participation (F < 2.410, P > 0.05). WHODAS 2.0 total scores negatively correlated with MBI score (r = -0.540, P < 0.001), except the scores of domains of Life Activities and Participation (r = 0.184, P > 0.05). Conclusion WHODAS 2.0 can be used as a tool to assess and follow up the function and disability of old stroke patients.
