脑蛋白水解物对高原地区缺氧缺血脑病新生儿血清超敏C反应蛋白、胰岛素样生长因子1 和白细胞介素-18 的影响①

Effects of Cerebroprotein Hydrolysate on Serum Insulin-like Growth Factor-1, High Sensitivity C Reactive Protein and Interleukin-18 in Newborn with Hypoxic Ischemic Encephalopathy in Plateau Region

摘要: 目的研究脑蛋白水解物对高原地区新生儿缺氧缺血脑病(HIE)患儿疗效及血清中超敏C反应蛋白(hsCRP)、胰岛素样生长因子-1(IGF-1)和白细胞介素-18(IL-18)水平的影响。方法2013 年1 月~2015 年5 月本科收治的HIE患儿104 例,随机分为常规治疗组(n=52)和脑蛋白水解物治疗组(n=52),另选取35 名健康新生儿作为对照组。观察治疗前后患儿血清中hsCRP、IGF-1、IL-18水平及临床疗效。结果与对照组和轻度HIE 组比较,中、重度HIE 组患儿IGF-1 水平下调(P<0.05),hsCRP、IL-18 水平上调(P<0.05)。治疗后,脑蛋白水解物治疗组总有效率大于常规治疗组(χ2=8.922, P=0.012)。与常规治疗组比较,脑蛋白水解物治疗组治疗后血清中IGF-1 水平上升,hsCRP、IL-18 浓度下降(P<0.05)。结论脑蛋白水解物对高原地区HIE 患儿疗效显著。血清hsCRP、IGF-1、IL-18 水平变化可作为HIE患儿早期诊断的辅助指标。

Abstract: Objective To investigate the effect of cerebroprotein hydrolysate on the serum high sensitivity C reactive protein (hsCRP), insulin-like growth factor-1 (IGF-1) and interleukin-18 (IL-18) in patients with hypoxic ischemic encephalopathy (HIE) in plateau region. Methods From January, 2013 to May, 2015, 104 cases with neonatal HIE in our department were randomly divided into conventional treatment group (n=52) and cerebroprotein hydrolysate group (n=52). Besides, 35 cases of healthy newborn were chosen as the control group. The total effective rate and the serum hsCRP, IGF-1, IL-18 levels were compared. Results The IGF-1 level was lower, and the levels of hsCRP and IL-18 were higher in moderate HIE group and severe HIE group than in the control group and the mild HIE group, respectively (P<0.05). The efficiency rate was significantly higher in the cerebroprotein hydrolysate group than in the conventional treatment group (χ2= 8.922, P=0.012). Compared with the conventional treatment group, the IGF-1 level increased, and the levels of hsCRP and IL-18 decreased in the cerebroprotein hydrolysate group (P<0.05). Conclusion Cerebroprotein hydrolysate is effective on patients with HIE in plateau region. The changes of serum hsCRP, IGF-1, and IL-18 levels can be used as auxiliary indexes for early diagnosis of HIE.
