Chinese expert consensus on comprehensive rehabilitation and non-invasive neuromodulation for prolonged disorders of consciousness

摘要:

目的 基于循证方法,形成对慢性意识障碍(pDoC)患者综合康复和无创神经调控促醒治疗的中国专家共识。
方法 成立共识核心工作组,结合国内外最新研究进展和临床实践经验,围绕pDoC患者的综合康复和无创神经调控促醒治疗两个核心议题形成初步推荐意见草案,经过两轮线上和线下专家咨询与讨论,所有条目最终推荐意见得票数超过70%,形成终稿。
结果 pDoC患者康复策略的制定需要以康复医师为主导,包括物理治疗师、作业治疗师、言语治疗师、康复护士、神经科医师、内科医师、营养师、社会工作者等在内的多学科协作。针对pDoC患者综合康复和无创神经调控促醒治疗,形成10条共识意见和6条推荐治疗参数。
结论 针对pDoC患者综合康复和无创神经调控促醒治疗的相关问题达成专家共识意见,有助于统一实施标准,推进临床康复规范化管理,为制定个体化康复策略提供参考。

Abstract:

Objective To develop a Chinese expert consensus on comprehensive rehabilitation and non-invasive neuromodulation awakening therapy for patients with prolonged disorders of consciousness (pDoC) using an evidence-based approach.
Methods A core working group for the consensus was established. Combining the latest domestic and international advancements with clinical practice experience, preliminary recommended opinions were formulated focusing on two core topics: comprehensive rehabilitation and non-invasive neuromodulation for pDoC patients. After two rounds of online and offline expert consultation and discussion, all the final recommended opinions received up to more than 70% of agreement.
Results The development of rehabilitation strategies for pDoC patients should be in team-work, led by rehabilitation physicians, involving physical therapists, occupational therapists, speech therapists, nurses, neurologists, internists, nutritionists, social workers, and other professionals. Ten consensus opinions and six recommended treatment parameters were developed for comprehensive rehabilitation and non-invasive neuromodulation for pDoC patients.
Conclusion The expert consensus on comprehensive rehabilitation and non-invasive neuromodulation for pDoC patients provides a standardized guidance to promote the standardized management of clinical rehabilitation, and provide references for developing individualized rehabilitation strategies.
