Abstract: Person Centred Planning(PCP) is a life planning approach designed for individuals with disabilities to increase their self-determination ability and improve their independence, which contains some commonly used methods, such as Personal Futures Planning,MAPS,Essential Lifestyle Planning,Whole Life Planning and Planning Alternative Tomorrows with Hope(PATH).It pursues social inclusion and designed to enable people to choose their own supports and services,by reflecting of the individual's personal communication mechanisms and assisting them to outline their needs, wishes and goals, rather than attempting to fit with the existed service systems. However,in the process, some people often adopt the form of this planning approach without the philosophical content. The complete solution of this problem need the active cooperation of relevant agencies even the entire community.
