Comparison of Two Analgesia in Early Rehabilitation on Patients after Knee Arthrolysis

Objective To compare the effects of continuous femoral nerve block (CFNB) and intravenous patient controlled analgesia(IPCA) on postoperative analgesia and rehabilitation therapy for patients after knee arthrolysis. Methods 60 patients were randomly allocatedinto CFNB group (n=30) in which patients received CFNB via a catheter inserted in femoral sheath and IPCA group (n=30) in which patientsreceived IPCA via an intravenous infusion pump contained with analgesics. All patients were successively offered passive rehabilitationtherapy and active rehabilitation therapy at 6, 24, 30, and 48 h post-operation. An injection of 50 mg ropivacaine (0.25%) was administratedvia the catheter before rehabilitation therapy in CFNB group. Analgesics (200~250 μg sufentanil and 100 mg ketamine in 100 ml normalsaline) were transfused intravenously at a constant rate during the whole postoperative period in group IPCA. Visual Analogue Score(VAS), active flexion angle (AFA) of suffered knee joint, and side effects related to CFNB or IPCA were recorded. Results The VAS scoreand the side effect rate were lower in CFNB group than in IPCA group (P<0.05). And the AFA was significantly larger in CFNB group thanin IPCA group (P<0.001). Conclusion CFNB is a safe and beneficial analgesia for patients in postoperative rehabilitation.

ObjectiveTo investigate the loss of heterozygosity (LOH) on 6q in bladder tumor.MethodsD6S404 and D6434 microsatellite markers near 6q21 were tested by PCR-SSLP-stain method on tumor DNA from 31 cases of bladder tumor.ResultsAmong these 31 cases of bladder tumor,LOH was detected in tumor tissues on site for D6S404 (35.5%) and D6S434(22.6%).ConclusionOne or more tumor suppressor gene near 6q21 maybe relevant for the development of bladder tumor.

The purpose of this study was to provide standards for healthy people performance on a Chinese version of the LOTCA.Fifty four the healthy and twenty five CVA patients participated in this study.The study was conducted in three phases.In phase one,the healthy subjects were assessed with the original version of the LOTCA.In phase two,the subjects who had made mistake in recognition of a typewriter for English in the sub test one,the healthy subjects were assessed with the original version of the LOTCA.In phase two,the subjects who had made mistake in recognition of a typewriter for English in the sub test of Categorization or had not known it at all were reassessed by the revised sub test of Categorization.In phase three,the LOTCA was administered to CVA patients.The results show significant differences between the healthy people and CVA patients.The score on the sub test of Categorization of the original version of LOTCA was much less than perfect,however the results of revised Categorization test was consistent with that from Katz et al.The results also show that years of education were related to perceptual cognitive performance.The LOTCA is a feasible and valuable tool,and is recommended for these individuals who has problems with cognition and perception.

Low back pain (LBP) is one of the most common types of disability affecting individuals in Western countries, and the assessment of LBP-related disabilities represents a significant challenge. The disability assessment scales in LBP are commonly used in clinical diagnosis and treatment as well as evaluation system for clinical research, such as Oswestry Disability Index (ODI), Roland Morris Disability Questionnaire (RMDQ), and Quebec Back Pain Disability Scale (QBPDS). This article focused on the clinical applications, reliability and validity, strength and limitations of these scales, to help the clinical application and scientific research for the LBP disability scale.

Kluver-Bucy syndrome (KBS) is a rare neuropsychiatric disorder. The characteristics of KBS include visual agnosia, hyperorality, changes in dietary habits, hypermetamorphosis, placidity, hypersexuality, and etc. However, some features like aphasia, amnesia,dementia and seizure are probably exclusive to humans. KBS is usually associated with lesions of the bilateral temporal lobe.

Objective To investigate the thermography characteristics in arterial lesions of lower limbs after type 2 diabetes mellitus.Methods 19 patients with arterial lesions of lower limbs after type 2 diabetes mellitus and 20 matched healthy controls were screened with digital thermography.The arteries of lower limbs of the patients were examined with the Doppler ultrasound.Results The patients' thermography showed abnormal cool regions or significant asymmetry between bilateral lower limbs or significant difference in temperature of bilateral lower limbs compared to the controls.The abnormal finding of thermography could be found in 100% patients,the difference of temperature between bilateral lower limbs could be found in 84% patients,while the abnormal finding of Doppler ultrasound in 89% patients.Conclusion Thermography can be used to screen the arterial lesions of lower limbs after type 2 diabetes mellitus,which is more sensitive than Doppler ultrasound.

ObjectiveTo determine the value of the five-times sit-to-stand test (FTSST) on assessment of physical performance for older people.Methods65 volunteers participated in this study. 21 of them were evaluated the FTSST twice by the same raters within one week. Test-retest reliability assessed using intraclass correlation coefficients (ICC). 65 volunteers were tested for the FTSST, the Timed "up & go" test(TUGT) and the strength of lower limbs. The Pearson's correlation (r) and stepwise regression analysis was used to examine the relationship between these variables and the FTSST time.ResultsThe FTSST showed good test-retest reliability(ICC=0-91, 95%CI:0-79~0-96). The TUGT, the strength of lower limbs and age were significantly related to the FTSST time (P<0-05). The strength of lower limbs was important clinical factor to consider in determining the FTSST time.ConclusionThe FTSST showed good test-retest reliability and can reflect the physical functional status of older people.

Nursing Care for Patients Accepting Pulse Steroid Therapy

Objective To establish the nursing care for the patients with pulse steroid therapy, and to investigate its effect. Methods 78 patients who accepted pulse steroid therapy in the past 4 years were analyzed retrospectively, in which, 42 within past 2 years (2008 and 2007) were as pulse steroid nursing care group, and other 36 patients before 2 years (2007 and 2006) were as routine nursing care group. Results 3 weeks after the pulse steroid therapy, in pulse steroid nursing care group, proximal muscle weakness were found in 8 patents, insomnia in 18 patients; while in routine nursing care groups, proximal muscle weakness were found in 16 patents, and insomnia in 25 patients(P<0.05). Conclusion Pulse steroid nursing care can reduce the incidence of muscle weakness and insomnia after pulse steroid therapy significantly.

Objective To apply Canadian Occupational Performance Measure (COPM) on the patients injured in Sichuan Earthquake. Methods COPM was applied to evaluate 51 patients in hospital before and 1 months after treatment. The first evaluation confirmed the problems on occupational activities for them. Then the plan was made to solve the problems. The second evaluation was to assess the effect of the treatment.Results The problem of self-care activity is more than that of productive activities and leisure activities(P<0.01). Total scores of performance of occupational activity and satisfaction improved(P<0.01, P<0.01)Conclusion COPM is helpful to confirm the problems of occupational activities and contribute to develope the primary goals for rehabilitation and treatment programs.

ObjectiveTo synthesize metal complex that exhibits superoxide dismutase (SOD)-like activity and determine its structure.MethodsThis chemical compound was synthesized by means of reflux in laboratory.Its structure was examined by infrared spectra (IR),nuclear magnetic resonance hydrogen spectra(H-NMR) and mass spectra (MS).The activity of the SOD was determine by xanthine oxidase (XOD).ResultsThe IR indicted that this complex showed a typical absorption band of -OH,C=N and benzene. The H-NMR reveals the existence of metal ion manganese (Mn3+ ,paramagnetic). The molecular weight was 365, which was obtained by EI-MS analysis. And this complex showed SOD-like activity.ConclusionA metal complex with low molecular weight and SOD-like activity has been synthesized.
